There are 4 main villages in the island, Astypalaia, Livadi, Maltezana and Vathi.

There 5 straight routes weekly to and from Piraeus port and daily connection to Kyklades and Dodekanese. There is also an airport in Astipalaia, with 4 flights weekly - straight connections to Athens and 3 flights weekly - connections to Athens from Rhodes via Leros and Kos.
